不過當這對雙胞胎到了16歲，喬治亞的ㄧ顆腎臟出現衰竭，需要洗腎，而偏偏她的腎屬於稀有類型，如果不進行移植手術，喬治亞很可能捱不到成年。深愛女兒的威爾此時毫不猶豫地要捐出他ㄧ顆健康的腎給女兒。
事情發展到此，似乎一切的難題都可以被迎刃而解。
不過接著凱也生病了。她也是十六歲，對威爾來說，凱跟喬治亞一樣都是他的寶貝女兒，但凱的腎臟也是屬於少見的類型。
而且，兩個女兒的病情發展都讓威爾沒有太多的時間可以猶豫該怎麼辦，他必須和時間賽跑，盡快做出決定。而目前看來，威爾的選擇有三個：
1. 想辦法在黑市買一顆腎臟？
2. 把自己一顆腎捐給雙胞胎中的其中一個。但問題是，要救誰？
3. 犧牲自己，把兩顆腎臟都給女兒們
乍看之下，威爾似乎只有三條路可以選，但其實威爾並不知道他還有第四個選擇。
他之所以沒想到是因為，這第四個選擇對他來說，實在太駭人聽聞了！
問題是，他的一對雙胞胎女兒目前正處於性命關頭的危急時刻，所以，這原本沒想到的恐怖選擇頓時成為了一項必須列入考量的選擇。(Kate Cheng)
